Here are some questions and answers to help you use the questionnaires

How do I answer the questions?
When you get to the page, there is some information about the questions and then you can decide if you want to answer them.
On each page, there is a question and some different options you can choose from to best describe how you feel.
You can read the question yourself or choose for the computer to read it for you, then you can choose one of the options.
Once you have picked your answer, click the blue arrow at the bottom of the page to go to the next question.

How do I answer the PHQ-9 and GAD-7 questions?
You can answer both these quizzes in the same way. First you go to the Mood Quizzes page here: Click Here
This will take you to a page with information about both quizzes. Choose which one you want to take, and then click either “click here to take the PHQ-9” or “click here to take the GAD-7”.

On each page, there is a question and then 4 different options about how often you have experienced something (e.g. sadness or anger) in the past two weeks. The options are No days, Some days, A lot of days, Nearly every day.

What will happen if I answer the questions?
The things you tell us are private and nobody else will know what you have told us. We won’t ask for your name, so we won’t know who you are. Because we don’t know who you are, we can’t tell anyone what you have said.
We do keep the scores that you give, so they can help us make our questions even better.
We will also ask about how hard you found it to answer our questions and how we might make them better.
You do not have to answer any questions you do not want to.
At the end, you can ask for a copy of your scores to be emailed to you. You need to give us your email for this, but we will only use this to send you your scores.
How do you keep my answers private?
We do keep the scores and feedback that you give, so they can help us make our questions even better. We do not know who you are or any information about you. This means your scores are completely private.
If you do give us your email address so we can send you a copy of your scores, we only use this email to send you your scores. We will not keep this email for any other use.
What if I want to share my scores with anyone else?
You are free to share your scores with anyone that you want to. We will not share your answers with anyone, but you can if you want to.
What if I need help to answer the questions?
You can ask a supporter, professional, friend or family member to help you to go through the questions if you want to.
You can also have the questions read out by the computer, if you want to.
